**Please be aware: ASHA CEUs may take up to 60 days after the Conference to appear on your transcript. If you do not see the your Conference on your ASHA registry after 60 days, please contact us, and we will follow up with ASHA.

Please do not resubmit a new form request for the same Conference, as this may cause delays and possible errors on your ASHA registry.

Please also note: ASHA does not allow us to include anything about being an ASHA CE Approved Provider or the number of ASHA CEUs earned on Certificates of Attendance. As long as you complete the below ASHA Continuing Education Request form, your information will be submitted to ASHA and put into their system.
