**Please be aware: ASHA CEUs may take up to 60 days after the SOS Training Course to appear on your transcript. If you do not see the SOS Training Course on your ASHA registry after 60 days, please contact us, and we will follow up with ASHA.

Please do not resubmit a new form request for the same SOS Training Course, as this may cause delays and possible errors on your ASHA registry.

Please also note: ASHA does not allow us to include anything about being an ASHA CE Approved Provider or the number of ASHA CEUs earned on Certificates of Completion. As long as you complete the below ASHA Continuing Education Request Form, your information will be submitted to ASHA and put into their system.
