ASHA CEU Participant Request Form

Per ASHA guidelines, participants who need ASHA Continuing Education Credits must complete the ASHA CEU Participant Form to receive ASHA CEUs for each SOS Advanced Workshop. If you are not requesting Continuing Education Credits for an SOS Advanced Workshop that you completed, you do not need to complete the form below.

**Please be aware – ASHA CEUs may take up to 60 days to appear on your transcript. If you do not see the SOS Advanced Workshop on your ASHA registry after 60 days, please contact us, and we will follow up with ASHA. Please do not resubmit a new form request for the same SOS Advanced Workshop as this may cause delays and possible errors on your ASHA registry.
